Follow-up Report Guidelines

Here are the questions asked during the Follow-up Report process:

Q1: Today’s date

Q2: Project Length (To Date)—Please enter the number of months (rounded to the nearest month) that the project has been active.

Q3: Activity Level—Has the project been inactive for any significant time?

Q4: Follow-up 1 (inactivity length)—If yes, then enter the number of months (rounded to the nearest month) that the project has been inactive.

Q5: Follow-up 2 (inactivity reason)—If yes, then please provide a brief explanation for the inactivity (e.g., summer break, staff turnover, etc.).

Q6: Project On Track?—In general, would you say that the project is on track as far as meeting the goals set forth in your grant application?

Q7: Project Hi-Lights—Please say a few words about your project “hi-lights” (e.g., successes, project goals met, etc.).

Q8: Project Lo-Lights—Please say a few words about your project “lo-lights” (e.g., setbacks, obstacles, delays, etc.).

Q9: Goal Corrections Needed?—Do you feel that any of the goals of the project need to be changed or modified (or even eliminated)?

Q10: Follow-up (describe corrections)—If yes, then please briefly describe how the project goals will be modified or changed (or even eliminated). Please mention if these changes will effect the budget and by how much.
